I've seen(and done) adding or taking away whole body parts, removing scars, changing the entire background, moving or changing hairstyle, adding or subtacting lots of weight, and even merging one person's head with someone else's "better body"
I do it for fun mostly, or to fix pictures where something is really wrong with the image, but it can be taken as far as you want it too. My family is always making jokes that none of us have had a pimple since I got photoshop [img]wink.gif[/img]
On the happy side of extreme retouching though , are things like restoring old family photos and adding color.

This one is a pinup I got in a 1945 magazine. It was lovely but I really wanted it in color suitable for framing, so I colorized and enhanced it. I removed the autograph since it would just be a duplicate anyway.

This is a picture of my great-grandparents sometime in the late 1920s, around the time they got married. It had been carried in my great-grandpa's wallet for more than 30 years and was wrecked.
